Probably a very old home...things like that aren't uncommon in them. I do computer work for an elderly couple with a beautiful old house from the late 1800s and there is a doorway like that at the top of their third floor....no door on it, though, just the doorway, but it is back from the top of the stairs in a similar way.

I don’t think it’s what happened here, given the landing and lower staircase, but sometimes this can happen if the house was originally built with a steep, ladder-like staircase, and then rebuilt with a more normal staircase which needs a longer run to make it to the top.
